• A full assessment is done on the client and caregiver on the initial intake

• Weekly follow-ups

• Home visits

• Vitals on individual clients / caregivers when needed

• Supervise personal support workers, home support workers, developmental service workers and social service workers

• Continue to educate and offer training to our staff

• We ensure our clients are aware of all the resources offered in the community
